Section Saf-C 515.01 - Application, Data Required(a) A registrant applying for a replacement registration plate shall furnish the following on the prescribed form:(2) Certification that the plate issued has been lost, stolen, damaged, or destroyed;(3) Whether the loss has been reported to a local police department and the name of such department;(4) The registration plate number and type; and(5) Signature of the registrant.(b) The application form shall be submitted, in person to a substation or authorized municipal agent or, in person or by mail, to: The Division of Motor Vehicles
