Section Saf-C 3309.06 - Issuance of State Labels(a) Labels shall be issued by the department.(b) Agencies shall order labels from the department by completing, signing, and submitting the "Request & Payment for New Hampshire Labels" form DSFM-115, revised 12/17.(c) Labels shall be sent to the agency, return receipt requested.(d) Upon issuing a label, the agency shall forward the label fee, paid for by the manufacturer in accordance with Saf-C 3309.11, to the department.N.H.
