Section Saf-C 212.11 - Application for Approval of Driver Attitude Program(a) Any person wishing to establish an approved driver attitude program shall submit to the commissioner an application containing the following information: (1) The name, address and telephone number of the organization and any affiliation or license necessary to satisfy Saf-C 212.10;(2) A complete description of the program as outlined in Saf-C 212.10(a);(3) A list of all instructors who shall be teaching as part of the program, including their names, dates of birth, addresses, educational background, and work experience; and(4) A complete description of the record system that the program will use to satisfy Saf-C 212.10(a)(7).N.H.
