Section Ret 502.13 - Filing of NHRS Forms 18 and 21, Employer's Notice and Certification of Termination(a) Every employer of an employee, police or firefighter member shall complete NHRS Form 18 when a member retires by providing the following:(1) The member's name, social security number and effective date of retirement;(2) The types of accrued leave credits and their value computed as the number of days paid times the member's daily rate of compensation;(3) The total amount of the member's termination pay;(4) Payroll data for the final 4 cycles of the member's employment;(5) An explanation for any delay on the payment of all final compensation to the member within 120 days of termination; and(6) A certification that the information is complete and accurate.(b) The certification in (a)(6) above shall contain the following: (1) The employer's name, address and telephone number;(2) The name and title of the certifying official,(3) An authorized signature; and(4) The date of completion.(c) Every employer of a teacher member shall complete NHRS Form 21 when the teacher retires by providing the following:(1) The member's name, social security number and effective date of retirement;(2) The types of accrued leave credits and their value computed as the number of days paid times the member's daily rate of compensation;(3) The total amount of the member's termination pay;(4) Contract payroll information for the final 4 contract periods of the teacher member's employment including the number of payroll cycles within each contract period, the contract salary and any additional compensation;(5) An explanation for any delay on the payment of all final compensation to the member within 120 days of termination; and(6) A certification that the information is complete and accurate.(d) The certification in (c)(6) above shall contain the following:(1) The employer's name, address and telephone number;(2) The name and title of the certifying official;(3) An authorized signature; and(4) The date of completion.N.H.
