Section Ret 206.01 - Petition for Declaratory Ruling

Requests for declaratory ruling shall be in the form of a petition and comply with Ret 202 and shall set forth the following information:

(a) The name, address, and telephone number of the petitioner;

(b) The relief requested by the petition; and

(c) A statement of the legal issue upon which a declaratory ruling is sought.
