Section Res 2607.02 - Form SJG-1a, Verification of Employment(a) A business seeking dispersion of grant funds shall supply on Form SJG-1a the following information:(1) Name, address and telephone number of the business;(2) Approved grant number, issued by the grant administrator;(3) A detailed description of the positions filled, including: b. Number of positions filled for each title;c. Amount of wages to be paid;d. Amount of benefits to be paid;e. Position start date; andf. Position termination date.(b) All information shall be typewritten.(c) The contact person representing the business shall:(1) Sign and date the form; and(2) Certify by his or her signature that all information provided on Form SJG-1a is correct and true.(d) The form shall be mailed or hand-delivered to: Department of Resources and Economic Development
