Section Res 216.02 - Requests for Rule Explanation Requirements(a) Each request for rule explanation shall provide the following information: (1) The name and address of the interested person; and(2) The rule number and title;(b) The request shall be addressed to the commissioner.(c) All information shall be typewritten.(d) The interested person shall sign and date the request.(e) The request shall be mailed or hand-delivered to: Office of the Commissioner
