Section Pos 1701.03 - Application

(a) Each applicant for a room and board scholarship under the scholarship for orphans of police and firefighters program shall make application on Form PEC-PFS, application for police and firefighters orphans scholarship, by supplying the following information:

(1) Applicant name;

(2) Applicant mailing address;

(3) Applicant present legal residence;

(4) Applicant telephone number;

(5) Enrollment information including:

a.Applicant college and address; and

b.Academic year in school for which assistance will be applied;

(6) Name and address of living parent or guardian;

(7) Name of deceased parent who served as a firefighter or police officer; and

(8) Place and date of death of deceased parent.

(b) Each applicant shall sign and date the application form.

(c) The parent or guardian of each minor applicant shall sign the certification section.

(d) The signatures shall certify that all information provided on the application is truthful.
