The effect of the applicant's signature on the examination application form shall be to certify that:
(a) The information provided on the form, and on any request for reasonable accommodation for disability in the administration of the examination, is true to the best of the applicant's knowledge and belief;(b) The photograph submitted is a photograph of the applicant; and(c) The document(s) submitted are genuine to the best of the applicant's knowledge and belief.N.H.
