Section Ph 401.05 - Duplicate/Replacement Original Certificate of Licensure or Renewal License - Issuance(a) If seeking a duplicate or replacement for an original certificate of licensure the applicant shall: (1) Submit a written request, signed by the pharmacist, to the board for replacement; and(2) Provide payment of the prescribed fee which shall be $50.(b) If seeking a duplicate or replacement for an annual renewal license the applicant shall: (1) Submit a written request, signed by the pharmacist, to the board for a duplicate or replacement; and(2) No fee shall be assessed for a duplicate or replacement renewal license.N.H.
