Section Ph 302.02 - Application(a) The preliminary application for reciprocal licensure may be obtained from a link provided on the NH board of pharmacy website or from the National Association of Boards of Pharmacy, 1600 Feehanville Drive, Mount Prospect, Illinois, 60056, telephone number (847) 391-4406, website www.nabp.net. This application shall be filed with the National Association of Boards of Pharmacy.(b) Following verification of the applicant's credentials by NABP the applicant shall receive an official NABP license transfer application in the mail.(c) The candidate shall file a completed NABP license transfer application provided by the National Association of Boards of Pharmacy along with NH form Ph A-1, revised September 2015, application for initial licensure as a pharmacist in NH, and attach the following: (1) A copy of the candidate's birth certificate, or if born outside of the United States, a copy of the certificate of naturalization or passport showing date of birth;(2) A recent, full-face photograph of the candidate attached to the application;(3) An official copy of the candidate's pharmacy college transcript mailed directly from the college to the board, or if a foreign graduate, certification from the FPGEC; and(4) The application fee of $265.N.H.
