Section Pari 806.28 - Signature Statement Required for Supplemental Application Forms

The signature of an applicant or an applicant's duly authorized representative required on supplemental application forms shall be on a statement preprinted on the form stating that:

(a) The information provided on the supplemental application form is true, accurate and complete; and

(b) The applicant acknowledges that, pursuant to RSA 641:3, making a false statement on this supplemental application form is punishable as a crime.
