Section Mirt 402.07 - Required Documents

Applicants for renewal of their licenses shall include with their application the following:

(a) A detailed report of the relevant circumstances if any answers to questions set forth in Mirt 402.05(d) and appearing on the application form is in the affirmative; and

(b) A photocopy of the applicant's national certification card or other verifiable document showing proof of certification.
