Section Mhp 404.01 - Reinstatement Requirements(a) An applicant wishing to reinstate his or her license shall: (1) Complete, sign and submit the "Reinstatement Application" form, dated June 10, 2014, no later than 6 months after the date of license expiration;(2) Attach documentation of 40 hours of CEs, at least 6 of which shall be in ethics and at least 3 of which shall be in suicide prevention, intervention, and postvention, taken within 2 years prior to the date of reinstatement: and(3) Pay the fee specified by Mhp 304.01(h).N.H.
