The "Criminal Record Release Authorization form" shall be sent by each current member listed on the "Board of Directors Member Listing" to the division of State Police at the Department of Safety in Concord, New Hampshire, along with a $25.00 check made payable to the "State of NH-Criminal Records", for each release form.
