Section Lab 1303.07 - Renewal License

(a) Every licensee shall file a completed form ELA at least 30 days prior to the expiration of the current license together with the renewal application fee.

(b) The applicant for a renewal license shall supply the information required in Lab 1303.03(c) (1)-(5) and (8).
