Section Inst 304.02 - Application for Approval(a) Any person wishing to establish an approved manufactured installation training and continuing education program shall submit a written request to the board.(b) The written request shall contain at minimum the following information: (1) Name address and telephone number of the organization and its affiliations and pertinent licenses;(2) Description of program; and(3) List of teaching instructors, including names and addresses and their educational and work experience background.(c) After an opportunity for a hearing, the board shall, when necessary revoke an approval for any course which does not follow this part.N.H.
