Section Inst 301.08 - Reporting Requirements(a) A licensee shall report any changes in address, name or other material changes in the conditions or qualifications set forth in the original application to the board office in writing no later than 10 days after the change.(b) Where appropriate, after proper application and payment of fee, the board's records shall be changed and a new license shall be issued for the unexpired term of the current license.N.H.
