Section Ins 2001.04 - Licensure Procedure(a) No utilization review entity, other than an exempt organization, shall perform utilization review services unless the entity has received a license from the commissioner.(b) To obtain a medical utilization review license, the utilization review entity shall make application to the commissioner by completing and submitting an application form prescribed by the commissioner for this purpose under Ins 2001.07. This application shall be accompanied by the appropriate application fee pursuant to Ins 2001.09.(c) Utilization review entities applying for a license shall provide complete answers to all questions appearing on the application form. If the space provided on the application form is not sufficient to provide the applicant the space needed for a complete answer, the applicant shall provide such additional information as is necessary to provide a complete answer on additional sheets of paper which shall be attached to the application form.(d) Each medical utilization review entity that has an application pending shall keep its application and any accompanying information or supporting material current. Any amendments or other changes to any of the documents that are part of or which accompany a pending application shall be filed within 30 days. Failure to comply with this requirement on the part of a medical utilization review entity shall be considered a violation of this part.(e) Any applicant whose application is rejected shall be entitled to appeal the denial in accordance with the provisions of RSA 541, RSA 541-A, and Ins 200, and shall be so notified.(f) Licenses issued pursuant to this part shall expire at the end of business on the March 31st following the date of issue. Such licenses shall be renewable pursuant to Ins 2001.05.N.H. Admin. Code § Ins 2001.04
