Every 2 years an applicant seeking license renewal shall complete and submit a "License Renewal Application" form, June 25, 2018, along with the school's annual filing fee in accordance with Hedc 304.06 at least 90 days prior to the expiration of the current license. Such renewal application shall comply with all criteria set forth in Hedc 303.01.
