Section He-Hea 305.01 - Filing Requirements
(a) Each applicant granted a certificate of need shall file an implementation report as required in RSA 151-C:12, V.
(b) Applicants shall complete and submit Form 305, "Implementation Report Subsequent to Certificate of Need Approval," effective September 2015.
(c) Implementation reports shall be filed according to the schedule below:
(1) Reports shall be filed semi-annually during the development stage of the project until commencement pursuant to He-Hea 301.12;
(2) Once a project has commenced, an initial report shall be filed within 30 days of such commencement; and
(3) Implementation reports shall be filed annually until the project is completed pursuant to RSA 151-C:12, IV.
(d) Applicants shall provide a response to all items in the report. Actual costs, not projections, shall be reported. Any item not applicable shall be so noted as "not applicable."
(e) An implementation report shall be considered to be filed if it is sent to the office of health services planning and review, department of health and human services and has been signed and dated by the chief executive officer of the facility in accordance with the requirements of He-Hea 206.01(b).
