Section He-Hea 301.14 - Form 301-C Application for a Change of Scope

An applicant for a change of scope to the originally issued CON including a waiver of change of scope under RSA 151-C:12, IV-a shall complete and submit form 301-C "Change of Scope and Waiver, " effective May 2014.
