Section He-Hea 2204.01 - Request for Applications

The board shall issue a request for applications (RFA) for robotic assisted surgery system services:

(a) Upon adoption of this chapter; and

(b) On July 1 of each year, provided that it has received a letter of intent that contains the following information:

(1) Name of the requestor that intends to file an application, including the names of all affiliates of such requestor;

(2) An outline of the proposed location of the new or additional robotic assisted surgery system and service;

(3) An outline, by zip code, of the proposed service area to be served by the service;

(4) A detailed description of the specific type of robotic assisted surgery system to be acquired; and

(5) An identification of any robotic assisted surgery system services in:

a. The proposed service area; and

b. Next closest service areas in all directions.
