Section He-Hea 1502.01 - Notification Process(a) Ambulatory surgical facilities that wish to participate in the 2-year pilot program established per 1996, 299, shall notify the board in writing of its intention to participate in the program.(b) The notice to the board shall state:(1) The name and address of the facility;(2) The intended start date of the program; and(3) The name and telephone number of a contact person for the program.N.H.
