Section He-W 577.03 - Provider Participation(a) All participating laboratory service providers shall: (1) Be licensed by the states in which they practice;(2) Be certified to participate in the medicare program if the provider is an independent laboratory;(3) Be Clinical Laboratory Improvement Amendments (CLIA) certified in accordance with 42 U.S.C. 263 a; and(4) Be an enrolled New Hampshire Medicaid provider.(b) If a laboratory test cannot be performed within the capabilities of the participating laboratory services provider, the specimens may be referred to another laboratory that is certified in the appropriate specialties and subspecialties of services in accordance with the requirements of 42 CFR 493.N.H.
