Section He-W 568.06 - Prior Authorization(a) The provider shall request prior authorization, as specified in He-W 530, for therapy services prescribed in excess of the limits described in He-W 568.04.(b) Providers of hearing aid services and devices shall request prior authorization in accordance with He-W 567.N.H. Admin. Code § He-W 568.06
