Section He-W 566.03 - Provider Participation Each participating dental provider shall:
(a) Hold an active license to practice dentistry in the state in which he or she practices;(b) Be a New Hampshire enrolled Title XIX provider; and(c) Request and obtain prior authorization from the department, in accordance with He-W 566.07.N.H. Admin. Code § He-W 566.03
