Section He-P 813.22 - Voluntary Closure of AFCR

When an AFCR no longer intends to operate, an oversight agency shall notify the department in writing of the following:

(a) The name of the AFCR;

(b) The certificate number of the AFCR;

(c) The address of the AFCR;

(d) The date the AFCR closed or will close; and

(e) The location that the resident(s) have moved to, including the name of the home and certificate number of the home, if available.
