Section He-E 801.32 - Provider Participation(a) All providers shall: (1) Be enrolled in NH medicaid as a CFI provider;(2) Meet the applicable licensing, certification, or other requirements of the specific service being provided; and(3) Comply with requirements contained in 42 CFR 441.301(c)(4).(b) All providers shall: (1) Create and maintain an individual provider care plan for each participant served in accordance with He-E 801.33(a);(2) Create and maintain other documentation in accordance with He-E 801.33 and as required pursuant to applicable state and federal law;(3) Submit claims for payment in accordance with He-E 801.34;(4) Provide services in accordance with this part, 42 CFR 455, 42 CFR 456, 42 CFR 431, and 42 CFR 1001; and(5) Be subject to monitoring and review by the department upon request.(c) All providers shall comply with the provisions of RSA 161-F:49 with regard to checking the names of prospective or current employees, volunteers or subcontractors against the state registry maintained by the department's bureau of elderly and adult services.(d) All providers shall report to the appropriate departmental authority any participant who is suspected of being abused, neglected, exploited, or self-neglecting, in accordance with the adult protection law, RSA 161-F:46.N.H.
