Section He-C 6448.21 - Annual Monitoring and Reporting(a) A completed form 2648 "Child-Placing Agency Annual Report" (October 2019) shall be submitted by a licensed child-placing agency to DCYF one year from the date of the initial issuance of the child-placing agency license and at the anniversary date for every year the child-placing agency continues to be licensed.
