Section He-C 6348.06 - Renewal of Certification and Notification of Changes for Enrolled Providers of Health Care and Laboratory Services(a) Health care and laboratory providers shall submit to DCYF a current copy of the verification of licensure by the appropriate state licensing board(s) at the time of license renewal.(b) Providers shall submit a copy of any renewed license verification to DCYF within 10 calendar days of its receipt.(c) Review of continued certification shall coincide with the date of expiration of the health care or laboratory provider's license.(d) For the period of certification, the provider shall notify DCYF in writing within 10 calendar days of any change in the information contained in the application and provide documentation of the change.N.H.
