Section He-C 6346.16 - Denial of Application and Revocation of Certification

An application shall be denied or provider certification revoked if:

(a) DCYF or DJJS determines that the state does not have a need for the service.
(b) The applicant or provider, or the individual acting on the applicant's or provider's behalf, submits materially false information to DCYF or DJJS.
(c) There has been a conviction for a felony or any crime against a child that has not been annulled or overturned.
(d) There has been disciplinary action taken by a licensing body or professional society, a finding of civil liability made for professional misconduct, or a finding of an ethical violation made by a state or national professional association or any other state's regulatory board.
(e) There has been cancellation of professional or general liability insurance by the insurance company.
(f) There has been abusive or neglectful treatment of a child as determined by any state statute.
(g) There has been failure to comply with He-C 6346.
