Section He-C 6346.08 - Notification of Changes(a) For the period of certification, the provider shall notify DCYF in writing within 10 days of any change in the information contained in the application and provide documentation of the change.(b) If not required by a state's licensing authorities a new employee's criminal records release authorization and central registry name search authorization shall be completed and forwarded to the DCYF certification specialist prior to providing direct services.(c) At the time of expiration of mandatory state licenses, the provider shall submit a copy of the renewed license to DCYF within 10 days of receipt.N.H.
