Section He-C 6343.15 - Notification of Changes(a) For the period of certification, the provider shall notify DCYF in writing within 10 days of any change in the information contained in the application or recertification application and provide documentation of the change.(b) At the time of expiration of mandatory state driver's licenses, private vehicle transportation providers shall submit a copy of the renewed license to DCYF.(c) Failure to notify DCYF of any change shall result in revocation of certification in accordance with He-C 6343.24 and denied payment.N.H.
