Section He-C 6340.16 - Denial of Application and Revocation of Certification(a) An application shall be denied or provider certification revoked if: (1) DCYF or determines that the state does not have a need for the service;(2) The applicant or provider, or the individual acting on the applicant's or provider's behalf, submits materially false information to DCYF;(3) The provider has been convicted of a felony or any crime against a child;(4) There has been abusive or neglectful treatment of a child as determined by any state statute; or(5) There has been failure to comply with He-C 6340.N.H. Admin. Code § He-C 6340.16
