Section He-C 204.04 - Format and Content of Motion(a) A motion for reconsideration shall include the following: (1) The exact legal name of each person moving for reconsideration and the residence address or principal place of business of the person;(2) A clear and concise statement of the reason(s) why the person believes the decision to be in error; and(3) A concise and explicit statement of the facts upon which the AAU is expected to rely in granting relief.(b) A motion for reconsideration may also include such other information as the party filing the motion deems relevant and reliable, and there may be attached to said motion exhibits, illustrations and sworn written statements.N.H.
