Section He-A 404.02 - Care Coordinator Responsibilities
(a) For each client, the assigned care coordinator shall:
(1) Meet with the client to complete the GPRA survey;
(2) Ensure that the client has had a clinical assessment and level of care recommendation completed within the preceding 90 days, or if not:
a. Conduct the assessment and make a level of care recommendation; or
b. Refer the client for an evaluation that includes an assessment and a level of care recommendation;
(3) Complete a service plan, including types of services, frequency of services, and the number of units of each service;
(4) Present the client with a choice of provider options based on the completed assessment and level of care recommendations;
(5) Obtain client signatures on any necessary consents;
(6) Issue electronic vouchers for appropriate client-chosen providers;
(7) Regularly review voucher utilization to determine whether services are being utilized;
(8) On a monthly basis:
a. Contact the client, either face-to-face or by telephone, to monitor progress, to address barriers and any additional needs, and to ensure client satisfaction; and
b. Conduct a service plan review and update the service plan as needed;
(9) On a quarterly basis, contact by phone, by electronic means, or in person, all treatment, recovery support, or peer recovery coaching providers and conduct a brief review of client progress;
(10) Require the client to complete a satisfaction questionnaire at one month, every 6 months thereafter, and upon discharge; and
(11) Meet with the client to complete the GPRA survey 6 months after enrollment in the ATR program and at the termination of ATR services, which shall be when:
a. The client or the service provider(s), or both, agree that the services are no longer needed;
b. The client is no longer eligible for the program;
c. The client is unwilling to participate in the program; or
d. The client has not been in contact with the care coordinator or a service provider for 60 days.
(b) Care coordinators shall comply with all applicable privacy and confidentiality laws, rules, and regulations.
(c) If the client chooses to receive ATR services from the same provider that provides care coordination for the client, the care coordinator shall require the client to complete a "Waiver of Alternative Provider for ATR Services" form (August 2011) .
