Section Fis 1101.01 - Multi-license Form
(a) A n applicant for any of the following resident license types shall complete and submit the "Application for Licenses" form, Rev 10/23, available at printable-license-app.pdf (nh.gov), to the department at 11 Hazen Drive, Concord, NH 03301, a regional office, or an agent location, in accordance with Table 1100.1 below, together with the fee as determined in accordance with Fis 1110.01:

(b) An applicant for any of the following nonresident license types shall complete and submit the "Application for Licenses" form, Rev 01/23, available at printable-license-app.pdf (nh.gov), in accordance with Table 1100.2 below, to the department headquarters or an agent location, together with the required fee as determined in accordance with Fis 1110.02:

(c) Every applicant shall also provide a copy of his or her current state issued motor vehicle driver license or state issued non-driver photo identification card which shall be used to determine if the applicant qualifies for status as a resident of New Hampshire, pursuant to RSA 214:11-a and Fis 1109.12.
(d) Every applicant for a firearm hunting license shall also present written proof of completion of a hunter education safety instruction course from this or any other state or Canadian province, or a previously held firearm hunting license issued by this or any other state or Canadian province, pursuant to RSA 214:23-a, I and Fis 1109.13.
(e) Every applicant for an archery hunting license shall also present written proof of completion of a bowhunter education safety instruction course from this or any other state or Canadian province, or a previously held archery hunting license issued by this or any other state or Canadian province, pursuant to RSA 214:23-a, II and Fis 1109.13.
(f) On the form required by (a) or (b) above, the applicant shall make the following affirmation, pursuant to RSA 214:18-b: "I certify that my privileges to fish, hunt, trap, guide, take lobsters, or engage in commercial marine fishing activities are not under suspension or revocation in any state or province, and further certify that I am aware that any conviction of certain felonies in any state or province prohibits the possession of a firearm or other dangerous weapon."
(g) On the form required by (a) or (b) above, the applicant may elect to make a voluntary contribution to the nongame program of the department or the wildlife legacy fund of the department, which shall be paid along with the fee for the requested license(s).
(h) If submitted to the department by mail to 11 Hazen Drive, Concord, NH 03301, the applicant shall also enclose a business sized, stamped, self-addressed envelope to allow return of the license document by mail.
