Section Env-Wt 905.13 - Signature and Certification(a) The individual who has been authorized by the organization to submit a request under Env-Wt 905.11, Env-Wt 905.14, or Env-Wt 905.16 or to submit information under Env-Wt 905.15 shall:(1) Sign and date the document containing the required information; and(2) Print or type his or her name and title on the document.(b) The signature provided pursuant to (a), above, shall constitute certification by the signer that:(1) The signer has been duly authorized by the organization to submit the request to the department;(2) The information contained in or otherwise submitted with the request is true, complete, and not misleading to the best of the signer's knowledge and belief; and(3) The signer, on behalf of the organization, understands that:a. The submission of false, incomplete, or misleading information constitutes grounds for the department to deny the request or to revoke any decision that is made based on the information; andb. The signer and the organization are subject to the penalties specified in New Hampshire law for falsification in official matters, currently RSA 641.N.H. Admin. Code § Env-Wt 905.13
