Section Env-Wt 309.04 - Information Required for Routine Roadway Maintenance Registrations(a) To register routine roadway maintenance activities, the responsible party shall provide the information in (c), below, to the department on the applicable Routine Roadway Activity Registration as listed in (b), below, that has been signed and certified as specified in Env-Wt 311.11.(b) The registrations required by (a), above, shall be as specified in Table 309-1, below: Table 309-1: Routine Roadway Maintenance Registrations

(c) The responsible party shall provide the following to the department in writing: (1) The name, mailing address, and web site URL, if any, of the person responsible for the activity and, if the person is other than an individual, the name, daytime telephone number including area code, and e-mail address of an individual who will serve as the person's point of contact for the activity;(2) The type of activity to be undertaken;(3) The location of the activity; and(4) The estimated start date for the activity and the anticipated length of time the activity will take to complete.N.H.
