Section Env-Sw 1506.01 - Application Filing(a) Applications shall be prepared: (1) On and with such forms as the department provides; or(2) If no form shall be provided pursuant to (1) above, on paper supplied by the applicant in a format consistent with the application content and format requirements specified in Env-Sw 1505.(b) Applications to certify a waste-derived product for distribution and use shall be filed with the department in triplicate.(c) Each copy of the application filed with the department shall bear an original signature pursuant to Env-Sw 1505.09.N.H.
