Section Env-Dw 811.15 - Additional Information Required for PWS Supplied by Groundwater(a) Subject to (b), below, the O/O of a PWS supplied by a groundwater source that is subject to Env-Dw 717 or Env-Dw 720 relative to groundwater monitoring and treatment shall include the following in the CCR or annual notice if required pursuant to Env-Dw 717 or Env-Dw 720: (1) An explanation of each significant deficiency identified by the department pursuant to Env-Dw 717 or Env-Dw 720 that has not been corrected; and(2) A list containing the information specified in (c), below, for each positive E. coli result.(b) The O/O shall inform the public annually until the significant deficiency is corrected or the groundwater contamination is addressed.(c) Each report required under (a) and (b), above, shall include, as applicable: (1) The nature of the significant deficiency and the date of discovery;(2) The source of the E. coli contamination, if known, and the date(s) of the positive sample results;(3) The health effects language in Env-Dw 804;(4) The date that the deficiencies or contamination issues were addressed;(5) How the deficiencies or contamination issues were addressed; and(6) For all unresolved issues, the department approved plan and schedule for correction, including interim measures, progress to date, and any completed interim measures.N.H.
