Section Env-Dw 1404.09 - Signatures and Certifications for Final Applications and Associated Documents(a) The applicant's authorized representative shall sign and date the final application as follows: (1) If the applicant uses a paper-based application, the authorized representative shall sign and date the application and print his or her name and title in the location indicated; and(2) If the applicant uses the web-based application, submitting the form shall constitute the signature and the date the submission is received by the department shall constitute the date of the application.(b) Submission of the application, whether electronically or on paper, shall constitute certification by the signer that: (1) The signer has been duly authorized by the applicant to sign the application;(2) The information provided is true, complete, and not misleading to the knowledge and belief of the signer;(3) The applicant understands that any department determination that the applicant and the applicant's project qualifies for funding from the PFAS RLF that is based on false, incomplete, or misleading information is subject to modification, up to and including reversal, through an adjudicative proceeding conducted in accordance with applicable provisions of Env-C 200; and(4) The applicant and the signer are subject to the penalties specified in New Hampshire law for falsification in official matters, currently RSA 641.(c) If the applicant's authorized representative is, or is acting on behalf of, a listed engineer as defined in Env-C 502.10, the signature also shall constitute certification that the signer understands that the submittal of false or misleading information constitutes grounds for debarring the listed engineer from the roster.N.H.
