Section Env-A 605.04 - Certification of the Responsible Official
(a) Each applicant, owner or operator shall appoint at least one person to be a responsible official, as that term is defined in Env-A 101.166.
(b) The applicant, owner or operator shall notify the department of the identity of the responsible official(s) by completing and filing an ARD-1 form signed by each appointed responsible official.
(c) Each application form, report, or compliance certification filed with the department pursuant to this chapter shall:
(1) Contain a written statement certifying the truth, accuracy, and completeness of the document; and
(2) Be signed by one responsible official.
(d) Unless otherwise specified, the certification required by (c), above, shall state that, to the best of the responsible official's knowledge and belief formed after reasonable inquiry, the statements and information in the document are true, accurate, and complete.
(e) Within 30 days of any change in the identity of a responsible official or applicant, the owner or operator shall file with the department an ARD-1 form containing the information specified in Env-A 1703.01(b)(1) through (b)(7) and the signed certification statement specified in Env-A 1703.01(c).
(f) Within 30 days of any change in the identity of the owner or operator, the new owner or operator shall file with the department an ARD-1 form containing the information specified in Env-A 1703.01(b)(1) through (b)(7) and the signed certification statement specified in Env-A 1703.01(c).
