Section Diet 405.04 - Administrative Obligations of Licensees

Licensees shall:

(a) Maintain their current business address, home address, phone number, and email address on file with the board's office; and

(b) Notify the board's office of any change in address, phone number, or email address no later than 30 days from the date of the change.
