Section Alc 313.03 - The Meaning of the Applicant's Signature The applicant's signature on the "Application for Initial Licensure as an Alcohol and Drug Counselor or Master Alcohol and Drug Counselor" form described in Alc 313.02 shall mean that:
(a) The applicant confirms that the information provided on the "Application for Initial Licensure as an Alcohol and Drug Counselor or Master Alcohol and Drug Counselor" form and that has been personally submitted to support the application is true, accurate, and complete to the best of their knowledge and belief; and(b) The applicant acknowledges that the knowing making of a false statement on the "Application for Initial Licensure as an Alcohol and Drug Counselor or Master Alcohol and Drug Counselor" form is punishable as a misdemeanor under RSA 641:3.N.H. Admin. Code § Alc 313.03
