Section 449A.690 - Submission of authorization to change form by agent must include declaration for agent to act on behalf of registrant

An authorization to change form submitted by an agent must include a declaration that the agent is legally authorized to represent or act on behalf of the registrant to make decisions concerning the health care of the registrant.
