Section 172-131-007 - RENEWALTo renew a license, applicants must meet the requirements of Neb. Rev. Stat. § 71-7448, §§ 71-7451 to 71-7453, and this chapter, including the following:
(A) Respond whether, since initial license or renewal, has any license of the facility in another state been revoked, suspended, limited or disciplined in any manner;(B) Pass any inspection that has been conducted prior to the renewal of the license;(C) If any misdemeanor or felony convictions of the designated representative, the supervisor of the designated representative and each owner of the licensee or any disciplinary action was taken against the licensee by another state, submit to the Department an official copy of the disciplinary action or court records, including charges and disposition; and(D) Attest that the information provided is true and correct to the best of their knowledge.172 Neb.
