Section 51-2-001 - FORM

Every license issued shall be in the form of a certificate which shall have thereon:

001.01 The name of the applicant.

001.02 The class of license.

001.03 The date on which issued.

001.04 The official name of the Board together with its seal.

001.05 The signatures of the members of the Board and the Director.
