Rule 38.5.2801 - LETTER OF TRANSMITTAL(1) The letter accompanying a telecommunications utility rate case filing to the commission shall contain a minimum of the following:(a) A list of documents submitted with the filing;(b) A list of names and addresses of those to whom copies of the rate case have been mailed;(c) A brief description of the proposed changes in service, rates, and revenue levels;(d) Reasons for the proposed changes;(e) Estimated number of customers whose rates will be effected;(f) List of names and phone numbers of utility employees or consultants who shall be responsible for answering questions concerning the rate case;(g) List of names and addresses of utility employees or consultants who should be included on the service list.Mont.
